New Student Registration

If you are new to our district:

Step 1: Download the Enrollment Packet from adams12.org.

Step 2: Complete the forms and gather the necessary documentation including:

  • Parent/Guardian Photo ID
  • Child's Birth Certificate
  • Current Proof of Residence
  • Immunization Records
  • Contact information for last school and most recent report card or class schedule
  • Most recent copy of IEP, 504 or ALP (if applicable)

Step 3: Prior to June 4, 2020, email your completed enrollment forms and all required documents to casey.sauer@adams12.org.  After June 4, 2020, email your completed enrollment forms and all required documents to choice@adams12.org.  

Step 4:  Complete the Post Registration Checklist and the course request survey appropriate for your student's grade level:

If your student's last school was within Adams 12 and you need to update your address proving you've moved into Century's boundaries:

Step 1: Complete a Household Change Form

Step 2: Email your completed form, parent/guardian photo ID and one current proof of residence to casey.sauer@adams12.org.  (Note: your email will not receive attention until the office re-opens at the beginning of August.) 

Step 3:  Complete the Post Registration Checklist and the course request survey appropriate for your student's grade level:

If your student's last school was within Adams 12 and has recently been approved to attend Century through the Choice process:

Step 1:  Complete the Post Registration Checklist and the course request survey appropriate for your student's grade level:

If you are a returning Adams 12 student, please complete the online check in process during the available window:

The online check-in process allows parents to confirm registration for returning students, at their convenience, from anywhere with an internet connection during the annual check in window generally held mid June through early August.
